Role Overview

As a Care Manager, you will manage the quality of care services while building a robust network of community contacts. Your primary measure of success will be the satisfaction levels of our clients and caregivers.


Key Responsibilities

Strategic Responsibilities

  • Develop and maintain relationships to create new business opportunities.
  • Contribute to the continuous improvement of operational systems and processes.
  • Foster a supportive environment for our caregiver team.
  • Ensure compliance with government accreditation standards.
  • Actively promote our mission and values to clients.
  • Innovate service delivery to meet client needs.

Salary and Benefits

  • Salary: $80,000 per annum plus superannuation.
  • Commission: Performance-based commissions available.
  • Work Schedule: Monday to Friday, promoting a healthy work-life balance.

Tactical Responsibilities

  • Create networking opportunities and attend events.
  • Review clients weekly and perform home/care assessments.
  • Develop care plans and manage client and caregiver issues.
  • Assist in recruiting caregivers and nurses.
  • Supervise care delivery and handle incident management.
  • Conduct orientation for new caregivers and clients.

Performance Measures

Achieving the following targets is critical to success:

  • Client Check-ins: Minimum of 35 phone calls per week to clients and caregivers.
  • Service Utilization: Maintain at least 65% utilization of planned service hours.
  • Client Retention: Aim for a termination rate lower than 2.5%.

Standards of Work

  • Carers should be effectively allocated and dispatched to minimize client issues and delays.
  • Client satisfaction should rate care plans at four stars or above.
  • Issues raised by clients or carers should be resolved within one day.

Qualifications

Mandatory:

  • Valid WA Driver’s License

Desirable:

  • Certificate IV in Training and Assessment
  • Certificate IV in Community Care
  • Graduate Diploma in Occupational Health and Safety
